Mandaluyong City Building Permit Application Form and Checklist for 2020

Are you eyeing for a new construction project soon? It doesn’t matter if it’s your dream house, a small commercial building, or a five-door apartment – you will need a Building Permit before you can start, well, building.

As a newbie on how construction works, we visited the City Engineering Department of the Mandaluyong City Hall to get an updated checklist of requirements for securing a Building Permit. Here are the documents we need to complete:

  1. 1 set Duly accomplished application forms (completely and properly filled-up)
    • Building Permit Box 1, Box 3A, Box 6, Box 7, Box 8, Box 9
    • Sanitary/Plumbing Permit Box 1, Box 6, Box 7, Box 8
    • Electrical Permit Box 1, Box 2, Box 3, Box 4, Box 5, Box 1a
    • Mechanical Permit Box 1, Box 6, Box 7, Box 8
  2. 6 sets Design Plans (Architectural, Structural, Electrical, Sanitary/Plumbing, Mechanical & Electronics) (signed by Owner/ Applicant, signed and sealed by duly licensed Engineers/ Architect; scale not less than 1:100mts)
  3. 2 sets Survey Lot Plan signed and sealed by duly licensed Geodetic Engineer
  4. 4 sets Specification signed and sealed by duly licensed Engineers/Architect
  5. 2 sets Structural Design & Computation (signed and sealed by Structural/Civil Engineer)
  6. 4 set Bill of Materials & Cost Estimate (signed and sealed by Engineers/Architect)
  7. 1 pc. Photocopy of PRC ID, PTR, & UAP-IAPOA (for architects only) of design professionals with 3-specimen signature and dried sealed
  8. 1 pc. Seismic Analysis (for 3-Story & higher)
  9. 1 pc. Boring and Load Test (for 3-Story & higher)
  10. 1 pc. Structural Certification & Analysis (signed & sealed by Civil/Structural Engineer, for a renovation that will affect the stability of the building)
  11. 1 pc. Transfer Certificate of Title T.C.T. (Certified True Copy, on file with the Registry of Deeds within the last 4 months) or Condominium Certificate of Title (C.C.T.)
  12. 1 copy Contract of Lease (in case the applicant is not the registered owner of the lot), or Deed of Absolute Sale
  13. 1 pc. Tax Declaration
  14. 1 pc. Current Real Property Tax Receipt
  15. 1 pc. Barangay Clearance
  16. 1 pc. Home Owners Assn. Clearance (if applicable)
  17. 1 copy Locational Clearance (City Planning Office)
  18. 1 pc. Secretary’s Certificate/Authority to Sign/Special Power-of-Attorney Authorization Letter (whichever is applicable)
  19. 1 pc. DENR – Environmental Compliance Certificate
  20. 1 pc. DOLE – Construction Safety & Health Program
  21. 1 pc. HLURB – Development Permit
  22. 1 pc. CAAP – Height Clearance
  23. Site Development Permit (issued by Sangguniang Panlunsod for Subdivision/Townhouses development)
  24. Provision of Batas Pambansa 344 (Accessibility Law)
  25. Earthquake Recording Instrumentation
  26. Rain Water Collector
  27. Traffic Impact (for high rise building & large scale construction)
  28. Contractor’s Tax (if under contract) or Affidavit of Non-Hiring of Contractor (if by administration)
  29. Comprehensive Public Liability Insurance & Group Personal Accident Insurance (accredited by the City of Mandaluyong)
  30. DOH Clearance (for outdoor antenna)
  31. Others that may be required

That sure looks a lot, isn’t it? Completing these requirements may take you a few days, and you might also need to pay fees for some documents. Remember, however, that securing a Building Permit guarantees the engineers, architects, and you (as the project owner) that everything complies to all the necessary standards, codes, and minimum requirements under the National Building Code of the Philippines (PD 1096), Accessibility Law (BP 344), the Fire Code of the Philippines (RA 9514), and other related building laws.
