Mandaluyong City Mechanical Permit Application Form and Checklist for 2020

If you’re planning to start a construction project in the city of Mandaluyong, Philippines, you need to secure a bunch of permits, documents, and requirements before doing any physical work – one of which is the Mechanical Permit. We saved you the time and money of visiting the City Engineering Department of the Mandaluyong City Hall yourself, see our checklist of requirements you need to complete for securing a Mechanical Permit.

Mechanical Permit Requirements

  1. 5 Sets Mechanical Plan (must be signed & sealed by PME and signed by the owner)
  2. 3 Copies Mechanical Permit Form (must be signed & sealed by PME and signed by the owner)
  3. Original copy of valid PRC ID & current PTR (must be signed & sealed)
  4. Photocopy of Contractor’s Tax
  5. Cost Estimate / Bill of Materials

Note: The Minimum size of the Mechanical Plan shall be 508mm x 762mm (20” x 30”) in 5 blue/white prints. All plans shall contain the following:

  • Air-condition, refrigeration, ice-making, freezer, chiller detailed plan with complete details, equipment schedule, and heat load computation
  • Ventilation: Fans and blowers with complete details and equipment schedule
  • Ductwork installations indicating duct sizes, dampers, control, filters, fireproofing, acoustic and thermal insulation, and capacities
  • Piping system: Assembly of pipes on racks and supports, terminal to terminal valves, fittings, sizes, and color-coding
  • Automatic Fire Protection Systems shall include riser diagrams, tapping points, locations of inspector tests and drain sites, as well as hydraulic calculations, design computation. Sprinkler, Fire Extinguisher, Fire Hose Cabinet
  • LPGas pipe and Kitchen fire suppression system, schedule, and specifications. For restaurant, gasoline station, others with the use of gas & LPG, you must submit an Affidavit of Undertaking (For the use of Liquefied Petroleum Gas & other Gasworks). It must be notarized, and signed by the owner.

Permit to Operate (PTO) Requirements

  1. Approved Mechanical Plan / As-Built Plan
  2. Mechanical Completion Plan
  3. Photocopy of Approved Mechanical Permit
  4. Original copy of valid PRC ID & current PTR (Must be signed & sealed)
  5. With ducting & ductworks installation for Ceiling Concealed and chiller type, you must submit an Air-balancing schedule from the contractor. (Must be signed & sealed by PME and signed by the owner)
  6. With gas works & LPG, you must submit original Leak Test Result and Certificate from the contractor. (Must be signed & sealed by PME and signed by the owner)

Just like the Electrical Permit, a Mechanical Permit is one of the required Technical Documents before the issuance of a Building Permit. As shown above, requirements may be more or less depending on your project, but there’s one thing you need to remember – incomplete documents will not be accepted.

Are you eyeing for a new construction project soon? It doesn’t matter if it’s your dream house, a small commercial building, or a five-door apartment – you will need a Building Permit before you can start, well, building.

As a newbie on how construction works, we visited the City Engineering Department of the Mandaluyong City Hall to get an updated checklist of requirements for securing a Building Permit. Here are the documents we need to complete:

  1. 1 set Duly accomplished application forms (completely and properly filled-up)
    • Building Permit Box 1, Box 3A, Box 6, Box 7, Box 8, Box 9
    • Sanitary/Plumbing Permit Box 1, Box 6, Box 7, Box 8
    • Electrical Permit Box 1, Box 2, Box 3, Box 4, Box 5, Box 1a
    • Mechanical Permit Box 1, Box 6, Box 7, Box 8
  2. 6 sets Design Plans (Architectural, Structural, Electrical, Sanitary/Plumbing, Mechanical & Electronics) (signed by Owner/ Applicant, signed and sealed by duly licensed Engineers/ Architect; scale not less than 1:100mts)
  3. 2 sets Survey Lot Plan signed and sealed by duly licensed Geodetic Engineer
  4. 4 sets Specification signed and sealed by duly licensed Engineers/Architect
  5. 2 sets Structural Design & Computation (signed and sealed by Structural/Civil Engineer)
  6. 4 set Bill of Materials & Cost Estimate (signed and sealed by Engineers/Architect)
  7. 1 pc. Photocopy of PRC ID, PTR, & UAP-IAPOA (for architects only) of design professionals with 3-specimen signature and dried sealed
  8. 1 pc. Seismic Analysis (for 3-Story & higher)
  9. 1 pc. Boring and Load Test (for 3-Story & higher)
  10. 1 pc. Structural Certification & Analysis (signed & sealed by Civil/Structural Engineer, for a renovation that will affect the stability of the building)
  11. 1 pc. Transfer Certificate of Title T.C.T. (Certified True Copy, on file with the Registry of Deeds within the last 4 months) or Condominium Certificate of Title (C.C.T.)
  12. 1 copy Contract of Lease (in case the applicant is not the registered owner of the lot), or Deed of Absolute Sale
  13. 1 pc. Tax Declaration
  14. 1 pc. Current Real Property Tax Receipt
  15. 1 pc. Barangay Clearance
  16. 1 pc. Home Owners Assn. Clearance (if applicable)

  22. 1 pc. CAAP – Height Clearance
  23. Site Development Permit (issued by Sangguniang Panlunsod for Subdivision/Townhouses development)
  24. Provision of Batas Pambansa 344 (Accessibility Law)
  25. Earthquake Recording Instrumentation
  26. Rain Water Collector
  27. Traffic Impact (for high rise building & large scale construction)
  28. Contractor’s Tax (if under contract) or Affidavit of Non-Hiring of Contractor (if by administration)

Mandaluyong City Sanitary-Plumbing Permit Application Form and Checklist for 2020

In hopes of checking how ready we are to finally get a Building Permit for our soon-to-rise two-story building, we offered a visit to the City Engineering Department of the Local Government of Mandaluyong. Upon looking at the checklist of requirements for issuing a Building Permit, we noticed that there were a bunch of Technical Documents or Permits we have to acquire before actually completing the requirements for a Building Permit. One of which is the Sanitary/Plumbing Permit, to which the requirements are divided into three as shown below:

Requirements for Sanitary/Plumbing Permit as per Sec. 302 BP 1096 & RNPC

  1. Filled Sanitary/Plumbing Application Form (including fixtures to be installed)
  2. Photocopy of PTR/PRC of Design and In-charge (Sanitary Engr. or Reg. Master Plumber)
  3. 5 sets of Plumbing/Sanitary Plans with Sign and Seal of SE/RMP

Sec. 302.8 PD 1096 Sanitary Documents

For deep well, water purification plants, water collection and distribution system, reservoirs, drainage and water systems, sewage/water treatment plants, malaria control structures, and sewage disposal system:

  1. Location Plan and Site Plan
  2. Detailed Plan and Layout
  3. Design Analysis and Technical Specifications
  4. Cost Estimate

Sec. 302.9 PD 1096 Plumbing Documents

For all plumbing installations, addition and/or alteration involving hot or cold water supply, fixtures, sewage drainage and vent system, storm drainage and sewerage system within or adjacent within or adjacent to the building:

  1. Location Plan and Site Plan of Minimum Scale 1:2000
  2. Plumbing Plans, Layouts, and Details of minimum scale of 1:50
  3. Legend and General Notes
  4. Isometric Drawings of the Systems
  5. Design Analysis and Technical Specifications

Sec. 1011 RNPC Provision of Grease Trap and Grease Interceptor (commercial such as restaurants, etc.)

Note: Septic Tank shall be located at least 1.5m from the adjoining private property and complies with “Annex E “of RNPC

Mandaluyong City Electrical Permit Application Form and Checklist for 2020

Just as how excited we were to start the construction of our soon-to-rise two-story home, there are a bunch of documents that we need to get ahold of before officially starting construction. At first, we thought we only needed to apply for a Building Permit, little did we know that to accomplish that, we also need to acquire an Electrical Permit.

We immediately headed over to the Mandaluyong City Hall, where we’re planning to build our home, and inquired about the requirements for an Electrical Permit. The City Engineering Department gave us a small paper along with a list of requirements we have to complete. These are all required under Article 1.2 of the Philippine Electrical Code 2009, by the way. Let me share it with you:

  1. Filled Electrical Application Form
  2. Photocopy of PTR/PRC of Design and In-Charge Electrical Engineer
  3. 5 sets of Electrical Plan With sign and Seal of PEE and in Appropriate Metric Drawing
  4. Scaled drawings with the following:
    • Location and Site Plans (this is not a vicinity map)
      • Bordering Areas showing public or well-known streets, landmarks and /or structure and owner’s land drawn to appropriate metric scale unless the extend into the areas concerned;
      • Location of Service Drop, Service Equipment, and Nearest Pole of utility company (MERALCO) furnishing electrical; location of the meter as well as sizes of service entrance wires, conduits, and service equipment.
    • Legend or Symbols (Refer to Appendix A – Electrical Symbols of PEC)
    • General Notes and/or Specification
    • Electrical Layout
    • Schedule of Loads
    • Fire Alarm Circuits
    • Design Analysis / Voltage Drop Calculations / Short Circuit Computations
    • Single-Line Diagram
